How it works
After you purchase LEDs at any retail location, save your receipt and mail it, along with this completed rebate form to:

Earn a 50 percent rebate on bulbs and fixtures
We’ll give you a 50 percent rebate up to $5 per bulb — with a minimum rebate payment of $10 — limited to 40 bulbs per customer.
You can also earn a 50 percent rebate up to $10 on LED light fixtures.
