To claim your refund, please complete the following form by Monday, Aug. 16, 2021. If funds remain unclaimed, they will be remitted to your state on Dec. 1, 2021. Please allow 30-45 days for processing from the time we receive your request.
Notifications received in our office after Monday, Aug. 16, 2021 cannot be processed and the funds can be claimed from the state after Dec. 1, 2021.
If the owner of the funds is deceased, the funds can be reissued to another recipient. In order to process these requests, the requestor must be a spouse, Executor or Personal Representative. The following documentation must be either emailed to firstname.lastname@example.org or faxed to 1-800-540-2486 for processing:
- Copy of the letter advising of the unclaimed funds
- Copy of Death Certificate
- Executor or Personal Representative Paperwork (if someone other than spouse is requesting funds)
- Letter from requestor noting relationship to the owner and advising Name and Address of new recipient
